When patients look into SkinPen microneedling, one of the first things they search for is before-and-after photos.
But a before-and-after photo only tells part of the story.
How many treatments did the patient have? How far apart were the treatments? When was the “after” photo taken? What was being treated? And how much improvement can realistically be expected?
These details matter because SkinPen results develop gradually, and there is no single treatment number that applies to every patient.
Published SkinPen results include patients who have undergone different numbers of treatments, while the clinical study for facial acne scars followed a specific series of treatments over time.
Here is what those results can actually tell us.
First, What Is SkinPen?
SkinPen is an FDA-cleared microneedling system.
It is cleared to improve the appearance of facial acne scars in adults aged 22 and older across all Fitzpatrick skin types. It is also cleared to improve the appearance of wrinkles on the neck in adults aged 22 and older with Fitzpatrick skin types II–IV.
During treatment, very fine needles create controlled microscopic openings in the skin. This initiates a natural healing and remodeling process.
That last part is important: remodeling takes time.
Microneedling results should not be judged based only on what the skin looks like immediately after treatment.
What Do SkinPen Before-and-After Results Show?
SkinPen’s published before-and-after gallery includes patients photographed after different numbers of procedures.
Some published examples show results after three treatments, while others show patients who underwent four, five, six, or more procedures.
This illustrates one of the most important things to understand about microneedling:
There is no universal number of treatments that produces the same result for every patient.
The number of treatments that may be appropriate depends on what we are treating, the severity of the concern, the patient’s skin, the treatment area, and how the skin responds over time.
A before-and-after image showing a result after several treatments also should not be interpreted as meaning that every patient needs that exact number of procedures.
Instead, these photographs can help demonstrate how results may develop across a treatment series.
What Does the SkinPen Clinical Study Show for Acne Scars?
For facial acne scars, we have more than before-and-after photographs to consider.
In the clinical study described in SkinPen’s patient labeling, patients underwent a series of three treatments spaced approximately four weeks apart.
This is one reason three treatments are frequently discussed when patients ask how many SkinPen sessions they may need for acne scarring.
But three treatments should not be interpreted as a guaranteed endpoint.
The appropriate number of treatments depends on the individual patient, and SkinPen’s patient information specifically recommends discussing treatment goals with your medical provider.
The study also followed patients after the treatment series rather than evaluating the result immediately after the final procedure.
That distinction is important because skin remodeling continues after treatment.
Can You See SkinPen Results After One Treatment?
Some patients may notice changes after their first treatment.
SkinPen reports that patients can notice improvement early in the treatment process, and its published information emphasizes that improvement can continue over the months following treatment.
However, seeing an early change does not necessarily mean the skin has reached its final result.
Think of the first treatment as the beginning of a remodeling process rather than an instant transformation.
This is especially important when treating acne scars, where the goal is gradual improvement in the appearance and texture of the scars rather than simply creating a temporary surface-level change.
What Happens After Three SkinPen Treatments?
Three treatments are particularly relevant when discussing facial acne scars because that is the series used in SkinPen’s clinical study.
SkinPen’s published clinical information reports continued improvement after the treatment series.
At six months after the final treatment, 90% of patients reported noticing improvement in the appearance of acne scars in the treated area.
This does not mean that every patient will experience the same degree of improvement.
It means that the treatment should be evaluated over time rather than judging the result immediately after treatment number one, two, or three.
For some patients, three treatments may produce the improvement they are looking for. Others may benefit from additional treatment depending on their scarring and goals.
What About Four, Five, or Six SkinPen Treatments?
SkinPen’s published before-and-after gallery includes examples from patients who underwent longer treatment series, including four, five, and six procedures.
That doesn’t mean everyone should automatically purchase six treatments.
It demonstrates something different: treatment plans can vary.
A patient treating more significant textural concerns may have a different plan than someone seeking a more limited improvement.
Similarly, acne scars can vary considerably in type, depth, distribution, and severity. One person’s treatment plan should not be copied simply because their before-and-after photograph looks impressive.
This is why we prefer to start with a realistic treatment plan, evaluate how the skin responds, and then determine whether additional treatment makes sense.
Why SkinPen Results Can Continue to Improve
One of the biggest misconceptions about microneedling is that the result you see immediately after the skin heals is the final result.
It isn’t necessarily.
SkinPen creates controlled microscopic openings that initiate the skin’s healing and remodeling response. The process occurs gradually rather than overnight.
SkinPen’s published patient information notes that skin improvement may continue for months after treatment.
This is why we don’t recommend judging your final outcome the morning after your appointment or even immediately after completing a treatment series.
Your skin needs time to respond.
What Does “80% Noticed Improvement” Actually Mean?
You may have seen the statistic that 80% of patients noticed improvement in the appearance of acne scars one month after treatment.
SkinPen currently reports this result in its published patient information.
It’s encouraging, but it is important to understand what statistics like this mean.
“Noticed improvement” is a patient-reported outcome. It does not mean that 80% of patients had their acne scars disappear, nor does it mean that every patient experienced the same percentage of improvement.
Clinical outcomes, patient-reported outcomes, and before-and-after photographs all provide different pieces of information.
We prefer to look at the full picture rather than use a single statistic to promise a particular result.
Does SkinPen Completely Remove Acne Scars?
No treatment should be presented as guaranteeing complete removal of acne scars.
SkinPen is FDA-cleared to improve the appearance of facial acne scars in adults aged 22 and older.
“Improve” and “remove” are not the same thing.
Acne scars also come in different forms. Their depth, type, location, severity, and the surrounding skin can influence how they respond to treatment.
Some patients may respond well to microneedling alone, while others may need a broader acne-scar treatment strategy.
That is why an evaluation of the scars themselves is more useful than simply deciding that everyone with acne scars needs the same package.
So, How Many SkinPen Treatments Do You Actually Need?
There isn’t one answer for everyone.
For facial acne scarring, the SkinPen clinical study provides a useful reference point: three treatments spaced approximately four weeks apart.
But we don’t believe that means every patient should automatically stop at three treatments or commit to a much longer series before seeing how their skin responds.
Instead, we look at several factors:
- What are we treating?
- How significant is the concern?
- What type of acne scars are present?
- What is your starting skin condition?
- How does your skin respond to treatment?
- What level of improvement are you hoping to achieve?
- Would another treatment approach be more appropriate for part of your concern?
The goal is not to reach a predetermined number.
The goal is to create a treatment plan that makes sense for your skin.
Why Before-and-After Photos Should Be Viewed Carefully
Before-and-after photographs can be useful, but they should never be treated as a guarantee of your own outcome.
When evaluating a SkinPen before-and-after image, look beyond the photograph itself.
Ask how many procedures were performed, how long after treatment the final photograph was taken, what condition was being treated, and whether the photographs were taken under comparable conditions.
Most importantly, remember that another patient’s result cannot predict exactly how your skin will respond.
Published SkinPen examples are helpful for understanding what may be possible, but your treatment plan should still begin with your own skin.
Our Approach to SkinPen at Coastal Wellness MD
At Coastal Wellness MD, we approach microneedling as an individualized treatment rather than simply selling a predetermined number of sessions.
We evaluate what you want to improve, your skin condition, your medical history, and whether microneedling is actually appropriate for your concern.
If we recommend a treatment series, we also want to evaluate your response along the way.
You may need additional treatments. You may not.
And if another treatment would make more sense for part of your concern, that should be part of the conversation too.
The goal isn’t to perform the most treatments. It’s to determine the right treatment strategy for your skin.
